Day City Hospice in Dayton, OH is seeking a compassionate Licensed Practical Nurse (LPN) to provide patient-centered hospice care. You will deliver direct nursing care, implement care plans based on assessments, and educate families about diagnoses and care plans.

Qualificações

• Graduate of an accredited Practical Nursing or Vocational Nursing program.
• Current LPN/LVN license in the state of practice.
• Minimum of three years of nursing experience.
• Hospice, home health, community health, or medical-surgical experience preferred.
• Current CPR certification.

Responsabilidades

• Deliver direct nursing care in accordance with state nursing practice standards.
• Implement and manage individualized plans of care based on comprehensive patient assessments.
• Document patient care accurately and timely.
• Educate patients and caregivers about diagnoses, treatments, and care plans.
• Collaborate with the interdisciplinary hospice team to coordinate care and address patient needs.
• Monitor symptoms and provide pain and symptom management.
• Evaluate patient responses to medications and treatments to ensure comfort.

What You'll Do

As a Hospice LPN, you will play an essential role in providing compassionate, patient-centered care by:

• Delivering direct nursing care in accordance with state nursing practice standards
• Implementing and managing individualized plans of care based on comprehensive patient assessments
• Documenting patient care accurately and timely
• Educating patients and caregivers about diagnoses, treatments, and care plans
• Collaborating with the interdisciplinary hospice team to coordinate care and address patient needs

• Evaluating patient responses to medications and treatments to ensure optimal comfort and quality of life

What We're Looking For

• Graduate of an accredited Practical Nursing or Vocational Nursing program
• Current LPN/LVN license in the state of practice
• Minimum of three years of nursing experience
• Hospice, home health, community health, or medical-surgical experience preferred
• Current CPR certification
• Strong communication, documentation, and organizational skills
• Valid driver's license and insured, reliable transportation
• A genuine understanding of and commitment to hospice philosophy and compassionate end-of-life care
