Job summary

Continuum Care Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team serving patients in the Tiffin and Fremont communities. This full-time, on-site role provides skilled nursing care and support to patients and families during their hospice journey.

You will collaborate with an interdisciplinary team, administer medications per the plan of care, educate families, and document care in the electronic medical record.

Qualifications

• Current Ohio LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Experience in hospice, home health, long-term care, or medical-surgical settings preferred.
• Strong assessment, communication, and documentation skills.
• Ability to work independently while maintaining teamwork.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care to hospice patients per care plan.
• Monitor patient conditions and report changes to the RN Case Manager.
• Administer medications and treatments as ordered.
• Document patient care accurately in the EMR.
• Educate patients and families on disease, medications, and comfort measures.
• Collaborate with physicians, nurses, social workers, and aides.
• Support patients and families with compassion and professionalism.
• Participate in care conferences and interdisciplinary team meetings.

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice team serving patients in the Tiffin and Fremont communities. This role provides an opportunity to make a meaningful difference by delivering high-quality nursing care and support to patients and families during their hospice journey.

As a Hospice LPN, you will work collaboratively with an interdisciplinary care team to ensure comfort, dignity, and exceptional patient-centered care.

• Provide skilled nursing care to hospice patients in accordance with the plan of care.
• Monitor patient conditions and report changes to the RN Case Manager and clinical team.
• Administer medications and treatments as ordered.
• Document patient care accurately and timely within the electronic medical record.
• Educate patients and family members regarding disease processes, medications, and comfort measures.
