Job summary

Day City Hospice in Dayton, OH is seeking a compassionate Licensed Practical Nurse (LPN) to provide patient-centered hospice care. You will deliver direct nursing care, implement care plans based on assessments, and educate families about diagnoses and care plans.

Qualifications

Graduate of an accredited Practical Nursing or Vocational Nursing program.
Current LPN/LVN license in the state of practice.
Minimum of three years of nursing experience.
Hospice, home health, community health, or medical-surgical experience preferred.
Current CPR certification.

Responsibilities

Deliver direct nursing care in accordance with state nursing practice standards.
Implement and manage individualized plans of care based on comprehensive patient assessments.
Document patient care accurately and timely.
Educate patients and caregivers about diagnoses, treatments, and care plans.
Collaborate with the interdisciplinary hospice team to coordinate care and address patient needs.
Monitor symptoms and provide pain and symptom management.
Evaluate patient responses to medications and treatments to ensure comfort.
