Job summary

Day City Hospice in Dayton, OH is seeking a compassionate Licensed Practical Nurse (LPN) to join our patient-centered hospice team. You will provide direct nursing care in line with state standards and develop individualized plans of care.

You will collaborate with interdisciplinary team, educate patients and families, monitor symptoms, and ensure comfort and quality of life. Strong communication and CPR certification are required; a valid LPN license is essential.

Qualifications

• Graduate of an accredited Practical Nursing or Vocational Nursing program.
• Current LPN/LVN license in the state of practice.
• Minimum of three years of nursing experience.
• Hospice, home health, community health, or medical-surgical experience preferred.
• Current CPR certification.
• Strong communication, documentation, and organizational skills.
• Valid driver's license and insured, reliable transportation.
• A genuine understanding of and commitment to hospice philosophy and compassionate end-of-life care.

Responsibilities

• Deliver direct nursing care in accordance with state nursing practice standards.
• Implementing and managing individualized plans of care based on comprehensive patient assessments.
• Documenting patient care accurately and timely.
• Educating patients and caregivers about diagnoses, treatments, and care plans.
• Collaborating with the interdisciplinary hospice team to coordinate care and address patient needs.
• Monitoring symptoms and providing effective pain and symptom management.
• Evaluating patient responses to medications and treatments to ensure optimal comfort and quality of life.

What You'll Do
