Job summary

Capital City Hospice in Mt. Vernon, Ohio is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under hospice standards.

You will work Monday through Friday, 8:00 AM to 5:00 PM, coordinating with an interdisciplinary team to support comfort and quality of life for patients and families. The ideal candidate is a graduate of an accredited Practical or Vocational Nursing program, holds an Ohio LPN license, has at least one year of medical-surgical experience, CPR, and a valid

Qualifications

• Graduate of an accredited Practical Nursing or Vocational Nursing program.
• Current LPN license in the state of Ohio.
• Minimum one year of medical-surgical nursing experience.
• Previous hospice or home health experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation meeting organizational requirements.

Responsibilities

• Provide direct patient care in accordance with physician orders and established care plans.
• Implement current nursing practices based on comprehensive patient assessments.
• Accurately and promptly document patient visits, interventions, and outcomes.
• Educate patients and families regarding diagnoses, treatment plans, medications, and disease progression.
• Collaborate with the interdisciplinary hospice team, including Hospice Aides, Social Workers, Physical Therapists, Occupational Therapists, Dietitians, and Clinical Supervisors.
• Monitor patient conditions and communicate changes appropriately.
• Provide pain and symptom management while evaluating patient responses to treatments and medications.
• Support patients and families with compassionate, patient-centered care throughout the hospice journey.

Position Summary
