Job summary

Hospice of North Alabama is seeking a compassionate LPN to join our team in Decatur, AL. This full-time role provides 1:1 patient care in a hospice setting, following care plans and state nursing rules, with team support available as needed.

Responsibilities include recording care, educating patients and families, coordinating with the hospice team, and managing pain and symptoms while participating in quality improvement activities.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the state.
• CPR certification is required.
• Must have a reliable, insured vehicle.

Responsibilities

• Provide hands-on 1:1 care to patients nearing end of life.
• Follow care plans and state nursing rules.
• Keep clear, timely records of care provided.
• Educate patients and families about illness and care plan.
• Coordinate care with hospice team and manage pain and symptoms.
• Participate in training and quality improvement activities.
