Job summary

Assured Hospice, part of the LHC Group and within the Optum family, is seeking an LPN in Hospice to provide quality patient care in the patient's home. The role includes skilled nursing under RN supervision, medication administration, and assisting with daily activities to promote self-care.

Responsibilities include wound care and reporting changes to physicians, with a commitment to compassionate care and a supportive work environment aimed at professional growth.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Performs skilled nursing services under the supervision of a staff RN daily.
• Administers prescribed medications and treatments as permitted by state regulations.
• Assists the physician and/or staff RN in performing specialized procedures as needed.
• Assists the patient with daily activities and encourages appropriate self-care daily.
• Reports to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed.
• Performs complex wound care if in-service is documented for specific procedure daily or as needed.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
