Job summary

Empath Health's Trustbridge Hospice in Palm Beach County is seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team. You will provide patient-centered care during daytime hours, with weekend availability, under the supervision of an RN and primary physician.

This role emphasizes comfort, dignity and full-life care. You will assess needs, develop care plans, monitor patients, teach families, and offer emotional support, helping ensure a respectful, personalized experience

Qualifications

• Current Florida LPN license.
• Current Basic Life Support (BLS) certification.
• 30-hour IV course completion is a plus.
• Adequate transportation with valid Florida driver's license.

Responsibilities

• Assess patient and family needs under RN and physician direction.
• Coordinate and implement individualized care plans prioritizing comfort.
• Use monitoring, interventions, and teaching to improve quality of life.
• Provide overnight emotional support to patients and families.
• Ensure personalized, compassionate service and positive experiences for patients and families.

Empath Trustbridge Hospice is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Palm Beach County area. You will work 8:30am - 5pm with weekend availability.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
