• Must hold a valid/active license as an LPN/LVN in Ohio.
• Prior hospice experience is highly preferred.
• Prior experience in community/home health and/or medical/surgical preferred.
• 3 years of nursing experience preferred.
• Complies with professional standards and practices.
• Demonstrates good verbal and written communication and organization.
• Must be able to lift, transfer patients and equipment.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work with the team to meet outcomes.
• Implement the plan of care initiated by the RN.
• Provide accurate and timely documentation aligned with the plan of care.
• Assess and educate patients and families on self-care and diagnosis-related topics.
• Coordinate home health and hospice services and report needs to RN/Clinic Manager.
• Use and prepare equipment adhering to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties as assigned, including home visits as census allows.
• Join weekend and holiday on-call rotation requirements.

Licensed Practical Nurse (LPN):

• Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
• Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
• Territory: ~30-Mile Radius (Mileage Reimbursed)
• Pay Rate: $28.00 - $31.00 per Hour
• Location:Reynoldsburg, OH & Surrounding Areas

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for home care patients as census allows.
• Participate in weekend and holiday on-call rotation requirements.
