Job summary

Summit Home Care & Hospice is seeking an LPN for Marysville, OH area. This role involves direct patient care, on-the-road visits, and responsibilities at the local office with occasional remote work.

You will follow care plans, document promptly, and educate patients and families. A valid Ohio LPN license and hospice experience are preferred, with consideration for community/home health backgrounds. Sign-on bonus available.

Qualifications

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as an LPN or LVN in Ohio.
• Prior hospice experience is highly preferred.
• Prior experience in community/home health and/or medical/surgical is preferred.
• 3 years of nursing experience preferred.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift and transfer patients and supplies.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care and document accurately.
• Assess and educate patients and families on self-care and diagnosis.
• Coordinate home health and hospice services with other disciplines.
• Maintain aseptic technique and proper equipment use.

Licensed Practical Nurse (LPN):

• Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
• Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
• Territory: ~30-Mile Radius (Mileage Reimbursed)
• Pay Rate: $28.00 - $31.00 per Hour
• Location: Marysville, OH & Surrounding Areas

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for home care patients as census allows.
• Participate in weekend and holiday on-call rotation requirements.

QualificationsQualifications:

• Prior hospice experience is highly preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• 3 years of nursing experience, preferred.
• Comply with accepted professional standards and practices.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift, position or transfer patients and lift supplies and equipment.
