Job summary

Summit Home Care & Hospice is seeking an experienced LPN to join our Home Health and Hospice teams in Worthington, OH. You will provide direct patient care on a Monday–Friday schedule with on-call rotation, traveling to patient visits within a ~30-mile radius.

The role includes mileage reimbursement, AI-assisted documentation support, and a comprehensive benefits package. Ohio license as LPN/LVN is required.

Qualifications

• Graduate from an accredited practical nurse program.
• Active Ohio LPN/LVN license.
• 3 years of nursing experience preferred.
• Home health/ hospice experience preferred.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and collaborate with the care team.
• Implement the plan of care initiated by the RN.
• Provide accurate and timely documentation of the plan of care.
• Educate patients and families on self-care per diagnosis and plan.
• Coordinate home health and hospice services with other disciplines.

Licensed Practical Nurse (LPN):

• Hours:Monday-Friday; Standard Business Hours& OccasionalOn-Call Rotation
• Work Setting:On the Road for Patient Visits& Training at the Local Office or Remotely
• Territory:~30-Mile Radius (Mileage Reimbursed)
• Pay Rate:$28.00-$31.00 per Hour
• Location: Worthington, OH & surrounding areas

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of careinitiatedby the registered nurse.
• Provideaccurateandtimelydocumentation consistent with the plan of care.
• Assess and provide patient and family/caregiver educationregardingself-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home healthand hospiceservices, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.
• Participate in weekendand holiday on-call rotation requirements.

QualificationsQualifications:

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold avalid/activelicense as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
• 3 years of nursing experience, preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• Comply withaccepted professional standards and practices.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift, position or transfer patients and lift supplies and equipment.
