Job summary

Summit Home Care & Hospice in Reynoldsburg, OH seeks a Licensed Practical Nurse (LPN) to provide direct patient care under RN supervision and implement the care plan during home visits. You will travel within a ~30-mile radius, with mileage reimbursement, and participate in on-call rotations with occasional remote training at the local office.

Ideal candidates hold an active Ohio LPN license, prefer hospice/home health experience, and demonstrate strong communication and organizational skills.

Qualifications

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
• Prior hospice experience is highly preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• 3 years of nursing experience, preferred.
• Demonstrates good verbal and written communication, and organizational skills.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for home care patients as census allows.
• Participate in weekend and holiday on-call rotation requirements.
