Job summary

Summit Home Care & Hospice is seeking an experienced LPN to support our Hospice & Home Health teams in Westerville, OH. The role includes patient visits, care under RN supervision, and collaboration with a multidisciplinary team to meet patient goals.

The ideal candidate has an active Ohio LPN/LVN license, hospice experience, and strong communication and organizational skills. This role involves travel within a ~30-mile radius and occasional on-call duties to cover weekends and holidays.

Qualifications

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
• 1 year of direct hospice experience; 3 years nursing experience preferred.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care and implement treatments.
• Document accurately and timely per the plan of care.
• Educate patient and family on self-care techniques and diagnosis-related care.
• Coordinate hospice services and report needs to the RN or Clinic Manager.
• Operate and prepare equipment, adhere to aseptic techniques.
• Participate in on-call rotations and weekend coverage.
