Job summary

Affinity Hospice seeks a Licensed Master Social Worker (LMSW) to assess psychosocial symptoms and develop objectives to alleviate distress for patients and families. The LMSW provides psychoeducation, counseling, and casework within the Interdisciplinary Group.

The role requires a Masters in Social Work, LMSW licensure in the state of employment, and experience with death, dying, and bereavement. Full-time, 40 hours per week with a focus on compassionate end-of-life care.

Qualifications

• Master's Degree in Social Work.
• Professional Licensure as a LMSW in the state of employment.
• Experience or training related to death, dying, and bereavement.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Assess psychosocial needs of patients and families regularly.
• Support pre-bereavement and bereavement care planning and referrals.
• Attend weekly IDG meetings to identify risks and changes in needs.
• Participate in on-call rotation with social work team.
