Affinity Health Management is seeking a licensed LMSW to implement standards of care for medical social work services to hospice patients and families. The role supports an interdisciplinary team, providing psychosocial assessment, planning, and crisis intervention in alignment with the established plan of care.

The LMSW conducts family counseling, assists in advanced directives, and maintains thorough clinical notes while coordinating with physicians and community resources.

Qualifications

• Master’s degree in Social Work (MSW) from a school accredited by the Council on Social Work Education.
• Minimum of one year of health care experience; hospice experience preferred.
• CPR certification required; licensed driver with insured vehicle.

Responsabilités

• Assess psychosocial status of patients and families; communicate findings to the care team.
• Conduct social evaluations including family dynamics; plan interventions; provide crisis intervention.
• Educate on advanced directives and participate in the plan of care.
• Assist in understanding social and emotional factors related to health problems and end-of-life issues.
• Maintain accurate clinical notes and coordinate care with physicians and community agencies.
• Participate in on-call duties and support case management and continuation of care.
