Job summary

Care 360 Hospice is seeking a Hospice Licensed Practical Nurse after-hours to support patients in Franklin County, Ohio. The part-time role includes Saturday night shifts from 8 PM to 8 AM, with travel to patient locations as needed.

Responsibilities include providing skilled nursing care, on-call after hours, and assisting RNs with the plan of care. LPN license, 1 year of skilled care experience, and valid driver’s license are required.

Qualifications

• Licensed Practical Nursing Degree is required.
• At least 1 year of skilled care experience.
• Hospice or home health experience preferred.
• Valid driver's license and auto insurance coverage.
• Reliable transportation and willingness to travel.

Responsibilities

• Provide skilled nursing care to patients in end-of-life settings, during visits in facility or home.
• Provide on-call services after hours and complete skilled visits.
• Assist RNs in carrying out the plan of care.
• Assist RN in performing procedures as directed by physician orders.
• Collaborate with IDGH to keep patient care plan current and address issues.
• Educate patients and families about end-of-life care.

Hospice Licensed Practical Nurse After-hours

Salary: $31.25-$34.13 BOE

Schedule: Part-time | Saturdays | 8 PM-8 AM

Location: Franklin County, Ohio, and surrounding area

Requirements of the Hospice LPN role:

• Licensed Practical Nursing Degree
• A minimum of 1 year of skilled care experience
• Hospice or home health experience preferred
• Valid driver's license and Auto insurance Coverage
• Reliable transportation: Willing to travel

About the Hospice LPN role:

• Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
• Provide on‑call services after hours, and complete skilled visits.
• Assist Registered nurses in carrying out the plan of care.
• Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist in the reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in‑service programs and other on‑the‑job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self‑development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
