Job summary

A community-focused hospice organization is seeking an After Hours RN/LPN in Greenville, Ohio. The successful candidate will be responsible for patient care after business hours, performing home care visits and supporting families through end-of-life care. The position requires a current RN or LPN license, a minimum of two years of experience, and excellent communication skills. Qualifications

• Experience in Medical/Surgical, Oncology or community health nursing preferred.
• Hospice experience preferred.
• Valid driver’s license required.

Responsibilities

• Provide competent nursing care to assigned patients through skilled symptom management and emotional support.
• Explain hospice services and benefits to patients.
• Order appropriate Durable Medical Equipment for patients.
• Facilitate continuity of care by coordinating admissions.
• Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
• Order appropriate DME for patients.
• Coordinate admission of home‑care patients to in‑patient settings as needed.
• Complete all documentation per policy and governing requirements.

Summary of Position

The After-Hours Nurse is responsible for direct patient care after business hours.

Education and Experience

• Graduation from an approved School of Nursing.
• Current RN or LPN licensure in the State of Ohio.
• Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
• Hospice experience preferred.

Special Skills and Requirements

• Shall have a valid driver’s license.
• Detail oriented with excellent communication, documentation, and organizational skills.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to assist with patient care activities such as ambulation, transfers, and positioning.

Responsibilities of Position

• Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs, including:
  • Routine homecare visits
  • End‑of‑life/continuous care
  • Referrals and admissions
  • Care center assistance
  • Chart reviews
  • Other duties as assigned
• Explain hospice services and benefits regarding Medicare, Medicaid and private insurance, and know when to use palliative care interventions and ABNs.
• Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
• Order appropriate Durable Medical Equipment (DME) for patients.
• Facilitate continuity of care by coordinating admission of home‑care patients to in‑patient settings (hospital, nursing home), as needed.
• Complete all documentation per organization policy, accreditation bodies and governing requirements.
• Other duties as assigned
