Job summary

Novant Health is seeking a Hospice Liaison Nurse to coordinate referrals and admissions, and to support transitions from acute care to hospice services. The role emphasizes collaboration with physicians, case managers, and interdisciplinary teams to ensure timely access to hospice care.

The position is hospital-based at Huntersville Medical Center, with travel to affiliated hospitals as needed. Strong nursing clinical skills and experience in hospice care are essential for success.

Qualifications

• Current RN licensure in the appropriate state.
• Bachelor's degree in nursing required; BSN preferred.
• Minimum three years' experience in home care or hospice.
• Experience with referral processing, admissions, and discharge planning is valued.

Responsibilities

• Process hospice referrals and coordinate admission scheduling.
• Conduct comprehensive clinical assessments to determine eligibility.
• Facilitate hospice admissions and transitions from hospital settings.
• Educate patients, families, and referral sources on hospice services and eligibility.
• Collaborate with physicians, case managers, and social workers to remove barriers to placement.
