Job summary

Halifax Health is seeking a Hospice Liaison to promote hospice, palliative and supportive care services within the community. The role serves as a liaison with physicians’ offices, hospitals and community groups, educating on Halifax Health‑Hospice and the continuum of care.

The ideal candidate has knowledge of hospice philosophy, Florida regulations, and healthcare sales/marketing experience. Strong communication skills and a valid Florida driver license are required to succeed in this role.

Qualifications

• Knowledge of Hospice philosophy of care; Medicare conditions of participation; State of Florida regulations governing hospice; hospice operations and sales and marketing.
• Healthcare experience, medical sales and marketing experience preferred.
• Good written, verbal and public speaking skills required.
• Possession of valid Florida driver license and clear driving record.
• Bachelor’s degree in related field or higher or equivalent years of experience.

Responsibilities

• Establish and maintain good working relationships with referral sources including physicians, hospitals, nursing homes, assisted living facilities and other healthcare professionals.
• Educate and be an ongoing resource of hospice, palliative and supportive care services offered by Halifax Health‑Hospice and the Halifax Health system.
• Educate and be an ongoing resource about continuum of care services.
• Develop plans of care for referral sources based on referral practices and barriers to early referral; maintain knowledge base.
• Act as liaison between referral sources and clinical operations to ensure customer satisfaction and patient outcomes.

Develop and maintain good customer knowledge base.* Act as liaison between referral sources and clinical operations of health system. Provide feedback to ensure optimal customer satisfaction and patient outcomes.* Ability to document customer compliments and complaints into the Halifax EMS system.
