Job summary

Halifax Health-Hospice is seeking a Hospice Liaison in Port Orange, FL to promote hospice and palliative care within the community. You will build relationships with physicians, hospital staff and other health professionals, educating them on continuum of care services and Halifax Health offerings.

The role requires knowledge of hospice philosophy, Florida regulations, and strong communication skills. A valid Florida driver license and healthcare or sales/marketing background are preferred.

Qualifications

• Knowledge of Hospice philosophy of care and Medicare conditions of participation; Florida hospice regulations.
• Healthcare experience, medical sales and marketing experience preferred.
• Strong written, verbal and public speaking skills are required.
• Valid Florida driver license with clean driving record.
• Bachelor’s degree in related field or equivalent years of experience.

Responsibilities

• Establish and maintain relationships with referral sources: physicians, hospital case managers, nursing home staff, and other healthcare professionals.
• Educate and be a continuing resource on Halifax Health-Hospice services for medical and non-medical communities.
• Educate about Halifax Health system services and hospice offerings.
• Develop care plans for referrals and identify barriers to early referral.
• Maintain knowledge base on services and referral practices.
• Serve as liaison between referral sources and clinical operations.
• Provide feedback to ensure patient satisfaction and outcomes.
• Document compliments and complaints in the Halifax EMS system.
