Job summary

Horizon Hospice Care is seeking a Hospice Community Liaison to strengthen relationships with physicians, discharge planners, and social workers, promoting Horizon Hospice services.

You will educate providers about hospice benefits, coordinate admissions with internal teams, and represent Horizon at community events while maintaining accurate outreach records.

Qualifications

  • Bachelor's degree in a healthcare-related field is preferred.
  • 1+ year in healthcare sales, marketing, or community relations.
  • Strong communication skills and ability to engage diverse stakeholders.

Responsibilities

  • Develop and maintain relationships with healthcare professionals to promote Horizon Hospice services.
  • Educate providers and partners about hospice and palliative care benefits.
  • Collaborate with internal teams to facilitate admissions and transitions of care.
  • Conduct outreach activities, presentations, in-services, and events to raise awareness of Horizon Hospice.

Hospice Community Liaison's Responsibilities:
  • Develop and maintain relationships with healthcare professionals, including physicians, discharge planners, case managers, and social workers, to promote awareness of Horizon Hospice services.
  • Educate healthcare providers and community partners about the benefits of hospice care and palliative services.
  • Collaborate with internal teams to facilitate timely admissions and transitions of care for patients referred to hospice services.
  • Conduct outreach activities, including presentations, in-services, and networking events, to raise awareness of Horizon Hospice within the community.
  • Act as a resource for patients, families, and community partners seeking information about hospice services and end-of-life care options.
  • Represent Horizon Hospice at community events, health fairs, and professional meetings to enhance our visibility and reputation in the community.
  • Maintain accurate records of outreach activities, referrals, and communications in accordance with company policies and regulatory requirements.

Requirements
  • Bachelor's degree in healthcare administration, marketing, business, or related field preferred.
  • Minimum of 1 year of experience in healthcare sales, marketing, or community relations, preferably in hospice or healthcare services.
  • Strong interpersonal and communication skills, with the ability to establish and maintain relationships with diverse stakeholders.
  • Knowledge of hospice regulations, Medicare/Medicaid guidelines, and healthcare industry trends is desirable.
  • Self-motivated and results-oriented, with the ability to work independently and as part of a team.
  • Proficiency in Microsoft Office suite and customer relationship management (CRM) software is preferred.
  • Valid driver's license and reliable transportation required.
