JourneyCare Hospice is seeking a Hospice RN to join our team in Highland Park, IL. This role offers competitive salaries, generous time off, a 401(k) with match, and a supportive team environment.

Responsibilities include managing patient and family caseload, coordinating with physicians and the interdisciplinary team, and providing regular visits and documentation to ensure high-quality end-of-life care. Candidates must be an RN with a state license and CPR/First Aid certification.

Qualifications

• Licensed as a Registered Nurse
• Graduate from an accredited RN program
• CPR and First Aid certifications are required
• Knowledge of quality assessment and assurance procedures
• Valid driver's license and proof of insurance

Responsibilities

• Manage patient and family caseload from assessment to evaluation phases of the nursing process
• Gather data from patient and family and coordinate care with interdisciplinary team
• Communicate with attending physician and agencies to coordinate care
• Provide regular visits to reassess patient needs and update care plans
• Perform clinical pronouncement of death and support families
• Maintain accurate records and care plans
• Supervise delivery of care by Home Health Aide per guidelines

HOURS: Monday-Friday 8:30am-5pm Plus One Weekend Every 12th Week (Saturday and Sunday) and One Summer and One Winter Holiday

TERRITORY: Highland Park, Lake Forest and Northbrook, IL.

Salary: $80,000-$97,000 Annually - Offer Based on Years of Experience

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
