Job summary

Joliet Area Community Hospice, founded in 1982, seeks an IPU Team Leader to provide clinical and operational leadership for RNs, social workers, CNAs, and chaplains. The role partners with the Director of Inpatient Unit to ensure safe, efficient care and alignment with budget goals.

The position requires a flexible schedule with 5 eight-hour shifts, including nights, weekends, and holidays, serving patients in home, facility, or inpatient settings as needed.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the State.
• Minimum of two years' experience in public health, home care, or hospice nursing is preferred.

Responsibilities

• Provide day-to-day clinical and operational leadership for RNs, social workers, CNAs, and chaplains in partnership with the Director of Inpatient Unit.
• Coordinate staffing, patient flow, clinical quality, medication stewardship, staff accountability, and education to meet budget and utilization goals.
• Direct report for CNA and Clerk staff including payroll, scheduling, attendance, discipline, development and education.
• Monitor care quality through chart reviews, rounds, IDT attendance, and feedback from physicians.
