Job summary

UPMC Family Hospice Aide IP Unit provides designated care under the direction of the primary service provider in the hospice inpatient unit (IPU) to help achieve patient care goals. The role involves rotating night and day shifts, weekend rotations, and 12-hour shifts with benefits offered by UPMC.

Responsibilities include active participation in agency meetings, process improvement, timely IPU team communication, direct patient care under supervision, and accurate documentation.

Qualifications

• High School Diploma or GED is required.
• Must have completed a training and competency evaluation program for Home Health Aide services.
• One year of experience is preferred; CNA preferred.

Responsibilities

• Participate in all required agency meetings.
• Identify, develop and participate in process improvement opportunities within the home health agency.
• Communicate with the interdisciplinary IPU care team in a timely manner.
• Provide safe and direct personal care to patients in the IPU setting under supervision.
• Complete all required forms accurately and in a timely manner.
• Maintain annual competency evaluation scores as required.
