Job summary

Lightways Hospice and Serious Illness Care in Joliet seeks an IPU Team Leader to provide clinical and operational leadership for a multidisciplinary team. You will manage staffing, patient flow, and quality initiatives to ensure safe, efficient care in the inpatient unit and home settings.

You will collaborate with the Director and Medical staff, participate in IDT meetings, and support medication reconciliation and staff development.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Bachelor's degree required.
• Hospice experience preferred.
• Previous management skills preferred.
• Minimum of two years' experience in public health, home care, or hospice nursing preferred.

Responsibilities

• Provide day-to-day clinical and operational leadership for RNs, social workers, CNAs, and chaplains.
• Direct report for CNA and Clerk staff: payroll, scheduling, attendance, discipline, development, education, evaluations.
• Prepare timely documentation for attendance, conduct, competencies, and staffing issues; escalate CNA performance concerns.
• Monitor quality of care through chart reviews, rounds, IDT meetings, and physician feedback.
• Coordinate staffing, patient flow, and budget/utilization targets with leadership.
• Review census and staffing levels; adjust schedules to meet budgeted costs.
• Support high-acuity IPU program development by identifying interventions and workflows.
• Lead weekly IDT meetings and medication reconciliation with physicians and nurses.
