Ohio’s Hospice, Inc. is seeking a Hospice PRN PCS in Dayton, OH. This field-based role involves traveling to patient homes and facilities throughout the Zanesville service region, delivering compassionate care with an interdisciplinary team.

The position is PRN traveling with day shifts, offering mileage reimbursement, company-provided technology, and opportunities for continuing education and professional development.

Qualifications

• High School Diploma or GED.
• Certified Nurse Aide (preferred, but will train internally).
• Certified Hospice and Palliative Nurse Aide (CHPNA) certification preferred.
• Proficient computer skills with the ability to document patient care and services in electronic medical records.
• Valid Ohio driver’s license with a safe driving record and proof of auto insurance.
• Ability to pass a criminal background check and 10 panel drug screening.

Responsibilities

• Deliver compassionate in-home and facility care as part of an interdisciplinary team.
• Travel to patient homes and care facilities across the Zanesville service region.
• Document patient care accurately in electronic medical records during shifts.

Hospice PCS – Making every moment matter

As a Hospice PCS, you will play a vital role in enhancing quality of life for patients and supporting families during some of their most important moments. Working alongside a dedicated interdisciplinary team, you\'ll deliver compassionate care that brings comfort, dignity, and peace. This is a field-based role requiring travel to patient homes and care facilities throughout the Zanesville service region.

This is a PRN traveling float position covering varied day shift hours. This is a driving position seeing patients in both facilities and homes within Ohio\'s Hospice South region which includes the greater Dayton area and all the surrounding counties.

Qualifications of this role:
