Job summary

Sequoia Hospice in Milpitas, CA is seeking an Intake Coordinator to manage the patient intake process, communicate with patients and families, and oversee insurance verification and referrals. This is a full-time, in-office position with a day shift and pay range of $27-$30 per hour.

You will coordinate with home health and hospice teams, ensure regulatory compliance, maintain relationships with referral sources, and support a smooth transition to home care.

Qualifications

• Healthcare experience in referrals/intake in hospice/home health preferred.
• Nursing background is preferred but not required.
• Excellent communication, negotiation and public relations skills are essential.
• Autonomy, assertiveness, flexibility and cooperation expected in performing duties.

Responsibilities

• Directs daily patient referral and intake operations and implements intake policies.
• Ensures compliance with state, federal, and Joint Commission requirements.
• Oversees improved methods to admit patients per policy.
• Maintains positive relationships with referral sources.
• Ensures insurance verification and authorization processes are properly handled.
• Monitors referral statistics and reports trends.
• Supports patient education and coordination of care for transition to home care.
