Job summary

CovenantHealth at Home with Compassus is seeking an Intake Coordinator for PAH to ensure smooth processing of referrals across home health and hospice programs. You will verify insurance, coordinate with clinical teams, and maintain accurate EMR documentation while fostering positive relationships with patients, families, and referral sources.

The role emphasizes prioritizing referrals, accurate data entry, and collaboration with clinicians.

Qualifications

• No formal education requirement.
• Minimum 1 year administrative experience in a home health or hospice environment.
• Preferred 1 year Home health or Hospice intake experience.

Responsibilities

• Prioritize daily referrals to ensure timely patient admissions.
• Accept referrals from multiple sources, including phone, fax, secure messaging systems, and other approved methods, ensuring accurate and timely entry into the EMR.
• Answer telephone calls promptly and courteously.
• Accurately transcribe medical and insurance information from referral sources.
• Verify and obtain physician orders, including validation of licenses and UPIN numbers.
• Evaluate insurance to ensure appropriate financial reimbursement.
• Verify coverage and communicate coverage criteria to patients.
• Collaborate with the clinical team to coordinate services.
• Assist with the orientation of new personnel to the department, as well as cross-department orientees, regarding referral processes.
