CarePartners Health Services, a CarePartners Health Services unit of HCA Healthcare, is seeking an Intake Coordinator for home health and hospice referrals. You will manage the full intake process, verify benefits, and ensure compliant documentation to enable timely admissions.

Strong cross-department coordination is required to support start-of-care timelines, patient experience, and referral partner satisfaction, with a focus on regulatory compliance and payer rules.

Qualifications

• Required Education: High School Diploma.
• Preferred Education: Bachelor’s Degree.
• Required License: None.
• Required Experience: 2-4 years of experience in home health, hospice, utilization management, or post-acute intake required.
• Preferred Experience: Familiarity with Medicare / Medicaid regulations in post acute care.

Responsibilities

• Manage full intake process for home health and hospice referrals.
• Verify benefits and ensure regulatory and payer documentation for admissions.
• Coordinate across departments to support timely start-of-care and accurate revenue capture.
