Job summary

Horizon Home Care & Hospice in Wisconsin seeks experienced nurses to join our Hospice Inpatient Unit. You will perform referral evaluations, admissions assessments, and comprehensive physical, psychosocial and spiritual assessments to guide patient and family care.

You will work within the IDG, ensure accurate documentation, and provide compassionate end-of-life support. Horizon offers a $10,000 sign-on bonus, benefits, orientation, and tuition reimbursement.

Qualifications

• Current Wisconsin RN license.
• BSN preferred; clinical nursing degree required.
• 1–2 years Hospice or oncology experience preferred.
• Experience in hospice care and end-of-life support.

Responsibilities

• Perform referral evaluations and admission assessments.
• Conduct physical, psychosocial and spiritual assessments.
• Communicate care goals and education needs to the health care team.
• Maintain clear, legible, timely documentation that meets standards.
• Provide Case Management as part of the IDG to an assigned caseload.
• Deliver services at professional standards with compassion.
