Job summary

Interim HealthCare Hospice is seeking a caring Hospice Registered Nurse (RN) to provide home-based care in Middleton, WI and surrounding areas. You will support patients with six months or less to live, focusing on comfort and quality of life.

Qualifications

• Graduate of an accredited nursing program and active RN license in Wisconsin.
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, and administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medications and supplies.
• Educate family on prognosis, care plan changes, medications and hospice goals.
• Take a holistic approach to care that engages patient, family, mind, body and spirit.
