Job summary

Horizon Home Care & Hospice in Mequon, WI is seeking experienced RN nurses to join the Lawlis Family Hospice Inpatient team. Part-Time (24 hours/week), 3 PM to 11:30 PM, with every other weekend and holiday rotation.

You will perform admission evaluations, psychosocial and spiritual assessments, document clearly for the health care team, provide case management, and deliver compassionate end-of-life care to patients and families.

Qualifications

• Active Wisconsin RN license required.
• Experience in Hospice or Oncology care is preferred.
• Compassionate, patient-centered bedside manner required.

Responsibilities

• Perform referral evaluations / admission assessments of patients and families seeking Hospice services.
• Conduct physical, psychosocial and spiritual assessments and document findings clearly.
• Communicate patient and family care goals to the health care team with accurate documentation.
• Provide Case Management services as part of the IDG for an assigned caseload.
• Deliver compassionate end-of-life care in line with professional standards.

We are seeking experienced nurses to join our Lawlis Family Hospice Inpatient team. Â

• Perform referral evaluations / admission assessments of patients and families seeking Hospice services. Â
• Performs physical, psychosocial and spiritual assessments utilizing non-verbal cues and probes or modifies questions to obtain more comprehensive assessments
• Communicates patient’s and family’s expectations of care, goals and comprehensive learning needs to all members of the health care team via appropriate documentation too
• All documentation is clear, legible, concise, accurate, timely, and meets department standards
• Provide Case Management functions, as part of the IDG, to an assigned case load.Â
• Provide services at quality levels consistent with professional standards of practice. Approach patients and families with care, concern and compassion.
