Horizon Home Care & Hospice in Wisconsin seeks experienced nurses to join our Hospice Inpatient Unit. You will perform referral evaluations, admissions assessments, and comprehensive physical, psychosocial and spiritual assessments to guide patient and family care.
You will work within the IDG, ensure accurate documentation, and provide compassionate end-of-life support. Horizon offers a $10,000 sign-on bonus, benefits, orientation, and tuition reimbursement.
