Qualifications

• BSN required.
• Current State of Illinois Registered Professional Nurse.
• American Heart Association Basic Life Support (BLS) certification.
• Strong interpersonal skills and ability to collaborate with others.
• Maintains an active driver’s license and auto insurance.
• ADN acceptable with commitment to obtain BSN within three years of first day of clinical nurse role.

Responsibilities

• Assess and record patient status and needs at admission or point of care.
• Organize health data for accuracy and accessibility.
• Identify problems needing immediate attention.
• Coordinate interdisciplinary care.
• Participate in joint home visits with the Charge Nurse.
• Document diagnoses and plan of care with measurable outcomes.
• Initiate discharge planning early and involve patient/family.
