Responsibilities

• Assess and record patient status and care needs at admission or point of care.
• Organize health data for accessibility and accuracy.
• Identify problems requiring immediate attention.
• Coordinate interdisciplinary care.
• Perform joint home visits with Charge Nurse.
• Validate diagnoses with patient/family and health team.
• Document diagnoses to support outcomes and care plan.
• Initiate admission plan and update as needed with age-specific care.
• Discuss and confirm the care plan with patient/family.
• Begin discharge planning at admission and involve patient/family.
• Identify patient teaching needs throughout care.
• Arrange conferences with patient/family and health team.
• Prioritize evidence-based nursing interventions.
• Provide patient/family education grounded in evidence.
• Document assessments, interventions, and patient responses.
• Review health data to update the care plan.
• Ensure revisions drive toward measurable goals.
• Communicate patient progress to family and team.
• Collaborate on progress/plan in multidisciplinary meetings.
• Make referrals as needed for continuity of care.
