Responsibilities

• Assess and record patient status; identify care needs at admission.
• Organize health data for accuracy, completeness, accessibility.
• Coordinate interdisciplinary care and discharge planning.
• Document diagnoses and update plan of care.
• Initiate and update the patient’s plan of care with measurable goals.
• Reassess nursing actions and communicate with patient/family.
• Present patient information at team conferences for feedback.
• Refer for continuity of care as needed.
