Job summary

- ADN acceptable with commitment to BSN within three years
- Preferred experience in specific clinical area
Responsibilities
- Assess and record patient status on admission or point of care entry
- Coordinate interdisciplinary care and plans
- Document diagnoses and progress toward outcomes
- Initiate discharge planning and educate patient/family
- Provide age-specific care and evidence-based nursing interventions
- Communicate with patient/family and health team members
- Conduct joint home visits with Charge Nurse
- Refer to appropriate care teams for continuity of care
