The role emphasizes care planning, patient education, end-of-life care, and collaboration with a multidisciplinary team. RN licensure, BLS/CPR, and a valid DL are required.

Qualifications

• 1+ years nursing experience
• Experience in acute or hospice care preferred
• RN license required
• BLS/CPR certification required
• Driver's license required

Responsibilities

• Maintains accurate and thorough documentation per Medicare guidelines.
• Collaborates with healthcare team to ensure high-quality patient care.
• Ensures compliance with local, state, and federal regulations.
• Participates in ongoing professional development and training.
• Assesses and reassesses patient needs to provide appropriate care.
• Educates patients and families on the plan of care and changes.
• Delivers nursing care to patients with complex health issues.
• Manages end-of-life care issues with sensitivity and professionalism.
• Administers and manages medications accurately and safely.
