Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Registered Nurse to join the Boca Raton Inpatient Care Center. The schedule is 7am–7:30pm (36 hours) with every other weekend, delivering end-of-life care with dignity and comfort.

In this role you coordinate care with the physician and interdisciplinary team, perform assessments, monitor conditions, provide patient and family education, and ensure timely interventions.

Qualifications

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with valid Florida driver’s license.

Responsibilities

• Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team (IDG).
• Conduct thorough assessments of patient and family needs.
• Ensure timely and coordinated delivery of interventions and services outlined in the care plan.
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.

What you'll Do:
