Chapters Health System in The Villages, FL is seeking a dedicated RN for Hospice House Days to assess patient needs, coordinate the Plan of Care with the IDG, and deliver compassionate palliative care. Candidates must hold an active Florida RN license, have hospice or inpatient experience, and be proficient with EMR/EHR systems.

The role requires strong English communication and ability to manage emotional stress.

Qualifications

Current Florida RN license is required.
Active single-state/compact license or multi-state license.
Minimum 1 year inpatient or hospice experience preferred for external candidates; internal 6 months of LPN experience with hospice/home health within an affiliate preferred.
Experience with an EMR/EHR system is preferred.
Ability to communicate in English (verbal and written).

Responsibilities

Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
Coordinates the Plan of Care reflecting evolving patient needs.
Educates patient, family, caregivers and other health professionals about disease, palliative interventions and safety practices.
Reports changes in the patient’s condition to IDG or other health professionals.
Participates with IDG to evaluate hospice referrals/admissions for level of care appropriateness.
Attends patient deaths and prepares body for transport; oversees disposal of medications.
Supervises care provided by LPNs and Hospice Aides.
Performs other duties as assigned.
