Job summary

Chapters Health is seeking an experienced registered nurse for Hospice House in Florida. The RN will assess patient and family needs, coordinate care with the IDG, and deliver palliative and supportive care in a compassionate setting.

Responsibilities include coordinating a patient-centered Plan of Care, educating families about disease progression and palliation, and communicating changes to the IDG. Strong English communication and flexibility are essential for this role.

Qualifications

• Current Florida license as RN or equivalent.
• Minimum 1 year inpatient nursing or hospice experience; internal may require 6 months LPN with affiliate.
• Experience with EMR/EHR systems.

Responsibilities

• Provide direct care to patients and families as part of IDG.
• Coordinate Plan of Care reflecting evolving needs.
• Educate on disease process, palliation, safety and caregiving.
