Job summary

Agrace seeks a CNA for Homecare to support patients and families with activities of daily living and compassionate, patient-centered care. The CNA will document care, monitor changes, and assist with mobility, hygiene, meals, and light housekeeping.

Qualifications include a state CNA certification, high school diploma, and a valid driver's license. Epic experience and Hospice/home health background are preferred. This 0.7 FTE day-shift role offers meaningful work in a nonprofit hospice setting.

Qualifications

• High school diploma or GED required.
• CNA certification in state of employment.
• 1 year of personal care or hospice/home health preferred.
• Valid driver’s license and auto insurance.
• Experience with electronic health records (Epic preferred).

Responsibilities

• Assist with activities of daily living.
• Assist with ambulation and positioning.
• Assist with bathing, dressing, grooming, and meals.
• Provide light homemaker tasks and housekeeping.
• Document care in the electronic health record and report changes.
• Take vital signs and assist with treatments.
