Agrace is seeking a skilled Certified Nursing Assistant for homecare in Madison, WI. This part-time 0.7 FTE position (28 hours/week) offers a day shift, Monday–Friday, 8:00am–4:30pm, with weekend and holiday rotations.

You will assist with activities of daily living, provide light homemaker help, document care, and follow the plan of care. Valid CNA certification, driver’s license, and EHR experience are required.

Qualifications

• High school diploma or GED, or equivalent combination of education and work experience.
• Graduation from an accredited Certified Nursing Assistant program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year personal care experience, hospice, or home health, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Basic Microsoft Office skills.
• Experience using electronic health records.
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsabilités

• Assist with Activities of Daily Living (ADL) and mobility.
• Provide light homemaker assistance and meal prep.
• Document care and communicate changes in patient condition.
• Read, understand, and follow the plan of care; report significant changes.
• Precept new staff and delegate tasks to volunteers.
• Participate in meetings to plan patient/family services.
