Job summary

Agrace, a leading hospice provider in Wisconsin, is hiring a CNA for day shifts in Janesville. This full-time position includes responsibilities in-home care, basic homemaking tasks, and following the care plan to support patients and families.

The CNA will document care in the electronic health record, assist with activities of daily living, and coordinate with the interdisciplinary team. Competitive pay and benefits accompany this role.

Qualifications

• High school diploma or GED or equivalent required.
• Graduation from an accredited CNA program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year personal care experience, hospice, or home health, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Basic Microsoft Office skills; experience with electronic health records (Epic preferred).
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.

Responsibilities

• Assist patients with activities of daily living and mobility, including toileting and incontinence care.
• Provide light homemaker tasks such as meal prep and housekeeping assistance.
• Follow the plan of care and document changes in the electronic health record.
• Communicate changes in patient condition to the care team and respond to patient needs.
• Participate in meetings and precept new staff and students.
