Agrace is seeking a compassionate Certified Nursing Assistant to join our hospice and palliative care team in Dodgeville, WI. This 1.0 FTE, daytime position supports patients in Activities of Daily Living and homemaker tasks, with weekend and holiday rotations.

You will document care in the EHR, communicate with the interdisciplinary team, and participate in ongoing staff development. Eligible candidates have a CNA credential and valid driver's license.

Qualifications

• High school diploma or GED or equivalent work experience.
• Graduation from an accredited Certified Nursing Assistant program.
• Current, unrestricted CNA certification in state of employment.
• One-year personal care experience, hospice, or home health preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Basic Microsoft Office skills.
• Experience using electronic health records.
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational IP.
• Ability to use proper body mechanics and standard precautions.

Responsabilités

• Assist with Activities of Daily Living including ambulation, toileting, bathing, eating, and transfers.
• Provide homemaker assistance such as light housekeeping, meals, and supplies.
• Document care plans, changes in patient condition, and report significant events in the EHR.
• Participate in meetings and precept new staff; delegate tasks to volunteers as appropriate.
