Agrace is seeking a dedicated CNA for day shift in the Madison area to provide compassionate homecare and hospice support. The role includes assisting with activities of daily living, light homemaker tasks, and documenting care per the plan of care.

Starting pay is $18.50 per hour with a $3,000 sign-on bonus, and a full-time schedule Monday–Friday 8:00am–4:30pm, with weekend and holiday rotation. Driving and reliable vehicle required for home visits.

Formación

• High school diploma or GED, or equivalent combination of education and work experience.
• Graduation from an accredited Certified Nursing Assistant program; valid CNA certification.
• Experience in hospice or home health is preferred; valid driver's license and vehicle required.

Responsabilidades

• Assist patients with activities of daily living including ambulation and toileting.
• Provide light homemaker services such as meal prep and light housekeeping.
• Read and follow the plan of care; document care in the electronic health record.
• Communicate changes in patient condition to the Interdisciplinary Team.
• Participate in meetings and precept new staff.
