National Church Residences Home Health seeks a compassionate Registered Nurse Case Manager or Licensed Practical Nurse to join our team in the Columbus area. You will coordinate care plans for hospice patients and provide skilled nursing care in patients’ homes.

Qualifications

• Must hold current RN or LPN license in the state of Ohio.
• Minimum two years LPN and RN experience; hospice or home health experience preferred.
• Strong care coordination and communication skills.
