Job summary

National Church Residences Home Health division in Central Ohio seeks an RN Case Manager to cover admissions and coordinate patient care for seniors. The role is full-time, Monday–Friday 8:30a–5p with occasional on-call weekends and twice-a-year holiday coverage.

You will develop, implement, and update individualized care plans, coordinate with physicians and the interdisciplinary team, and educate clients and families about care options and end-of-life planning.

Responsibilities

• Assuring the development, implementation, and updates of the individualized plan of care, including communication with physicians and integration of orders.
• Develops individualized plan of care with involvement of the client, caregiver, and client representative and provides education, mentoring, and support throughout the plan of care; notifies of necessary plan changes.
• Uses health assessment data to determine problems, goals and interventions; coordinates with community health providers and the interdisciplinary team to establish and monitor home health eligibility.
• Completes an initial assessment of client, caregiver, and client representative to determine home care needs.
• Initiates appropriate preventive and rehabilitative nursing procedures; may administer medications and treatments as prescribed by the physician/extender.
• Communicates with the physician/extender regarding the client’s needs; prepares clinical notes and updates the primary physician when necessary.
• Provides direct client care as defined in the State Nurse Practice Act.
• Educates and mentors the client, caregiver, and client representative in providing care per plan of care; supports end-of-life care discussions.
• Regularly re-evaluates client nursing needs and revises the plan of care as needed.
• Coordinates discharge planning with interdisciplinary members and attends team meetings to coordinate care plans.
• Responsible for instruction, evaluation, plan development, and supervision of LPN/LVN and HH/Hospice aides per policy and competency requirements.
