Job summary

National Church Residences seeks a Licensed Practical Nurse to provide nursing care and case management to clients in home-based settings in Columbus, OH. The LPN will implement the care plan, document care, educate families, and coordinate with RNs and other disciplines.

The position requires an LPN license and at least 12 months facility/agency experience, with travel in the service area and a valid driver's license. Non-Exempt, full-time role with potential on-call responsibilities.

Qualifications

• Education: Degree in nursing with LPN license.
• Experience: 12 months facility/agency nursing preferred (LPN).
• Travel: Valid driver’s license and automobile insurance required.

Responsibilities

• Provides direct patient care per State Nurse Practice Act and physician orders.
• Implements plan of care initiated by RN.
• Documents accurately and timely per plan of care.
• Educates patient and family about diagnosis and plan of care.
• Coordinates home health services and communicates needs to RN Case Manager.
