Job summary

CHI Health at Home is seeking a Hospice/Home Health RN to serve Nebraska City, delivering compassionate, patient-centered care in the home environment. You will lead care plans, collaborate with a multidisciplinary team, and educate patients and families.

You will mentor LPNs/LVNs and Home Health Aides, leverage advanced documentation tools, and ensure accurate, timely records while maintaining a strong focus on quality and safety. A 1:1 patient focus with autonomy is emphasized.

Qualifications

• Active RN license in the state served.
• Minimum 1 year in nursing; home health is a plus.
• Strong critical thinking and case management skills.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide high-quality nursing care in patients' homes.
• Develop and adjust comprehensive care plans.
• Collaborate with physicians, therapists, and staff.
• Educate patients and families.
